Individuals 18 years of age or older and their dependents are eligible for coverage. Senior citizens may also apply for coverage.
Routine preventive services (cleanings), basic services (fillings), major services (such as oral surgery/crowns) and orthodontic services will be covered ─ depending on the plan selected.* An optional rider allows you to increase your maximum annual benefits for a monthly fee. In addition, an optional vision rider will provide 85% coverage for vision exams and 50% benefits for lenses and frames. Contact lenses are covered 50% if purchased in lieu of glasses.
*Waiting periods and deductibles will apply to certain services and some major services are covered to a predetermined percentage.
